Personally instead of this:

I use just 2 pole cable, but you have to make a little cut with knife on the notebook side of cable.

Just get ground loop isolators from radio shack to hook up from your laptop to mixer and you will be fine

1) some notebook adapters have 2 plug power cord by default
2) many amplifiers have "ground breaker" switch to avoid ground looops
3) I also prefer and highly reccomend only ground loop isolators, but when you come lastminute or have to improvise there's a way and notebook will be grounded via the audio cable. And yees, it is also possible you will blow your soundcard... I had a luck and it's working
4) ok. I'm a fool :-)

I have a numark cm200usb mixer and the only cables between the laptop and mixer are 2 usb cables. How can I remove the ground loop noise (and still use ground)?

Are you using a laptop? If so then use a ground isolation conntector, it is easy to find at any hardware store. Turns a three prong wall male connection to a two pronged male connection.

No I believe they are talking about the Ground Loop Isolator that goes on the RCA cables from the mixer to the amp. I have used these in noisy computers and they work great. There is also a product called the Hum X.
