HandBrake is favoured by many. It's open source, available for Windows/Mac/Linux and the quality is awesome. What more could you ask? :)

I've used DVD Decrypter for most of my rips, but Handbrake is a good tool as well.

Rights to DVD Decrypter were seized from the author by Macrovision some time back, unfortunately, making it hard to find. Has the status of this changed? The author subsequently released imgburn to replace some of its functionality, but I'm pretty sure it doesn't handle the CSS.
